Frequently asked questions
All new patients (and established patients who have not been to our office within the past year) need to complete the Patient InformatIon Form. For your convenience, you may complete our Patient Information Form prior to your visit. You can print the form and fill it out by hand and fax it to us prior to your appointment (fax# 210.614.0530) or bring it with you when you come for your appointment. Please arrive at least 20 minutes prior to your scheduled appointment time. Be sure to bring a current insurance card and photo ID with you.

To schedule an appointment, please call our front desk @ (210) 614-3355. Our staff is happy to assist you in any way and every effort will be made to accommodate your schedule.
If you are unable to keep a scheduled appointment, please notify the office as early as possible. This appointment time has been reserved for you and failure to cancel your appointment within 24 hours (at least 1 business day) will regretfully result in a missed appointment charge.
As a service to patients, our office will bill the patient’s insurance (both primary and secondary) for services rendered. Co-payments, deductibles and payments for treatment not covered by insurance are payable at time of service.
We accept cash, check, Visa/MasterCard, American Express and Discover.
If your insurance company is an HMO and requires you to be referred to a specialist by your primary care physician, the referral must be received by our office prior to your appointment. Please call our office the day before your scheduled appointment to verify that we have received your referral.
