What Information Do I need to Provide?2019-03-21T04:18:37+00:00

All new patients (and established patients who have not been to our office within the past year) need to complete the Patient InformatIon Form. For your convenience, you may complete our Patient Information Form prior to your visit. You can print the form and fill it out by hand and fax it to us prior to your appointment (fax# 210.614.0530) or bring it with you when you come for your appointment. Please arrive at least 20 minutes prior to your scheduled appointment time. Be sure to bring a current insurance card and photo ID with you.

Click on the “Patient Information Form” link to download our 2-page form (PDF format). Save it to your hard drive, then fill out the form. Please bring a copy with you to your appointment. You may also download the Notice of Privacy Policies and Practices.

To schedule an appointment, please call our front desk @ (210) 614-3355. Our staff is happy to assist you in any way and every effort will be made to accommodate your schedule.

What is your cancellation policy?2019-03-21T04:20:35+00:00

If you are unable to keep a scheduled appointment, please notify the office as early as possible. This appointment time has been reserved for you and failure to cancel your appointment within 24 hours (at least 1 business day) will regretfully result in a missed appointment charge.

What is your insurance and payment policy?2019-03-25T20:11:12+00:00

As a service to patients, our office will bill the patient’s insurance (both primary and secondary) for services rendered. Co-payments, deductibles and payments for treatment not covered by insurance are payable at time of service.

We accept cash, check, Visa/MasterCard, American Express and Discover.

If your insurance company is an HMO and requires you to be referred to a specialist by your primary care physician, the referral must be received by our office prior to your appointment. Please call our office the day before your scheduled appointment to verify that we have received your referral.

Established in 1983, the Dermatology and Laser Center of San Antonio™specializes in general dermatology for adults and children, including detection and treatment of skin cancer, cosmetic, and laser procedures.
